Annual vaccine clinic

Saint Philip’s annual Vaccine Clinic will be held from 8:30am-11:30am in the West Murphey Gallery this Sunday, October 12.

Print and fill out this two page form and bring it with you, along with your health insurance card(s). Doing this will save time and allow the process to move along faster.

The following vaccines will be offered:
– standard dose for flu
– quadrivalent high-dose for adults over 65
– the current COVID vaccine
